One Final Kindness
These Terms and Conditions form the agreement between One Final Kindness Ltd, trading as One Final Kindness, and you (“the client”) when you book an appointment with my veterinary end-of-life service. By confirming an appointment, you acknowledge that you have read and accepted these Terms and Conditions.
One Final Kindness provides home euthanasia services for companion animals in Mid/South Wales. I cover up to 90 minutes travel from Tregaron, as measured by Google Maps at the time of booking. This is generally all of Ceredigion, Carmarthenshire, South Powys, South Gwynedd and North Pembrokeshire. I may be able to travel further if required but will discuss travel costs when contacted.
My service is limited to planned home visits for end-of-life care and euthanasia. I do not provide routine veterinary treatment, emergency care, diagnosis, surgery, prescriptions or ongoing veterinary services.
Home Visits
The client is responsible for providing safe access to the property and ensuring that the environment is suitable for the procedure. If there is any risk to the safety of me or my staff, I reserve the right to postpone or cancel the appointment.
Your pet will only be considered under my professional care from the time I, the vet arrives at your property until the euthanasia has been completed. Where cremation has been arranged through me, your pet remains in my care until transferred to the crematorium.
By booking an appointment you confirm that:
* You have the legal authority to request euthanasia, as the legal owner, or with the owner's full authority to make decisions.
* You have provided accurate information regarding your pet's medical condition.
Appointments are subject to availability.
While every effort will be made to attend at the agreed time, unforeseen emergencies, traffic or weather conditions may rarely cause delays. If I anticipate being significantly delayed, I will contact you as soon as possible.
Cancellations
If you need to cancel or rearrange an appointment, please let me know as soon as possible. I understand that circumstances can change. Where an appointment is cancelled with less than 24 hours’ notice, a £100 cancellation fee may apply, particularly where medication, travel or cremation arrangements have already been made.
Fees and Payment
The fees owed will be confirmed when your appointment is booked. Payment is due in advance or on the day of the appointment.
Where cremation has been arranged through One Final Kindness, the cremation fee will also be payable at this time.
Cremation Services
If requested, I can arrange cremation through a dedicated pet crematorium. Depending on your location, I work with a few different crematoriums.
Individual and communal cremation options are available depending on your preferences.
Ashes will ONLY be available to be returned where individual cremation has been selected. Ashes can be collected from the crematorium or hand delivered by us to your home, the cost of which will depend on your location and be discussed at booking. I do not post ashes.
Data Protection
I collect and process personal information solely for the purpose of providing veterinary services, maintaining clinical records, arranging cremation where requested, and complying with legal obligations.
Your information will be handled in accordance with UK data protection legislation.
These Terms and Conditions are governed by the laws of England and Wales.
Any disputes shall be subject to the jurisdiction of the courts of England and Wales.
